We have an audit tool built into Radar with 100 questions. There is no weighting on any questions and the options when completing are Adequate, Needs Attention, or N/A. When a question is marked as Needs Attention, the overall score should drop meaning, if 5 questions need attention, the score should be 95%. However, these audits are still being scored at 100%. Has anyone encountered this before and have any suggestions? As mentioned, there is no weighting on any of the questions in the tool.
