Create a Consent Form
Creating a consent form is now simpler and more flexible. You can search for and create consent forms for any medical procedure.
1. Add procedures
Search for the procedures you need to consent for. The search now uses SNOMED CT UK procedure names, so some procedure names may look different from previous versions.
2. Choose BRAN
Select either:

Recommended BRAN using EIDO's procedure-specific content, or

Custom BRAN using specialty-specific content.
3. Add patient information
Provide the right information for your patient by combining:
